Immunization Guidelines

NJ School Health Service Guidelines recommend a complete physical examination at least once childhood (pre school-grade 3), and pre- adolescence (grades 4-6). Our school district is now using the Universal physical examinations. If you plan to have your child examined, you may pick up a form in the Main Office of the school, or request completed by the parent/guardian, and Section II is to be completed by your nurse whenever your child has a physical examination.

Please be aware of new immunization requirements, in place since September 2008, for children in PreK, childcare and 6th grade, aimed at preventing influenza, pertussis, pneumococcal disease and meningitis.

Every child 12-59 months of age enrolling in or attending child-care or PreK must have:
a. Pneumococcal Conjugate Vaccine (PCV): at least 1 dose on or after the 1st birthday.
b. Influenza Vaccine: 1 dose between Sept. 1 and Dec. 31 of each year.

Every child born on or after Jan 1, 1997 and entering 6th grade on Sept. 1st must have:
a. Tetanus, diphtheria, acellular pertussis vaccine (Tdap): 1 dose, given on or after the 10th birthday
b. Meningococcal-containing vaccine: 1 dose

Families without health insurance can contact (732-341-9700, ext. 7612) for low cost any child from birth through 18 yrs. of age.

Families with limited or no health insurance cost from the Ocean Health Initiative (OHI). OHI is a federally funded program that provides comprehensive health services, including physical examinations and immunizations for children. They have offices in Toms River (732-552-0377) and Lakewood If your child is due for any of the above immunizations, please contact your healthcare provider for an appointment, to insure that your child meets the requirements at the appropriate times.
